Summer Reminders from the Health Office
If your child plans on participating in sports during the 2010-2011 school year please click on the above links for the Wellness Exam Forms that need to be brought to the doctor's office. If you have questions regarding why a well exam is needed please click on Frequently Asked Questions for Parents. Summer is a great time to have the well exam performed versus trying to get in to the doctor's office in the fall. Please return Well Exam form to the Health Office.
Please note the immunization schedule for 7th grade students as required by Vermont Immunization Law.
1) A new requirement that students have received two doses of Varicella vaccine (or present evidence of having had Chickenpox disease.)
2) A new requirement that students in the 7th grade have received a booster dose of Tdap.
Your child’s immunization record should look like the following:
· 5 DTaP/DTP
· 4 Polio
· 2 Measles
· 2 Mumps
· 2 Rubella
· 3 Hepatitis B
· 2 Varicella – waived if the parent or guardian presents a Department–supplied form indicating the student has a history of the disease.
· 1 Tdap booster
Exemptions are allowed if there is a medical contraindication to a specific immunization or if the student or parent/guardian claims a moral or religious objection to immunization. Please request an exemption form from the Health Office if needed.
Free immunizations may be obtained by contacting Kari Welling, Public health Nurse, at 885-5778 for an appointment at the Public Health Office at 100 Mineral Street, Suite 104, in Springfield, Vermont.
If your child has already received their immunizations please send documentation to the Health Office. Your doctor’s office may fax records to 463-9738.
